Gathering Information
J-Pak, Inc. may request and/or collect personal information that you elect to provide to us ("Personal Information") when you (1) register with us, (2) send questions or comments to us, (3) purchase items and/or (4) submit your resume to us online. The types/categories of Personal Information we collect may include: contact information (e.g., name, address, phone number and email address), birth date, credit card and ID verification information, such as credit card number, date of birth, Social Security Number, and Driver’s License Number, certain preferences (an example would be the types of packaging you use) and information regarding how you became aware of the J-Pak, Inc. In such instances where you want us to remove your Personal Information from our database, you merely need to contact us by: (a) visiting us at www.j-pak.com and filling out an email to us requesting removal of your Personal Information; (b) by visiting the specific J-Pak, Inc. Web site at which you registered and filling out an email request on that web site requesting the removal of your Personal Information; or (c) calling us at (812)346-9769 and asking for Customer Relations. J-Pak, Inc. will then take all reasonable efforts to delete your Personal Information from the J-Pak, Inc. database within a reasonable time frame, but, please be advised that J-Pak, Inc. may retain some residual information about you in our backup and archival copies of our database (e.g. our records of deletion) to send you important service-related communications (e.g. materials related to your purchases and/or your account) that are not promotional in nature.
